Position Summary

Designs, analyzes, interprets and coordinates quality improvement activities. Acts as a Performance Improvement Specialist with Colorado Health Neighborhoods (CHN) Affiliate Providers. Oversees compliance with meaningful use measures. Assists in identifying effective practices for transfer across the region and facilitates the redefinition of practice parameters, which improve quality, service and reduce costs.

_Minimum Education Requirements_

  • Bachelors Degree in Health Sciences, Business Administration required

_Minimum Experience Requirements_

  • 3 years health care and/or quality improvement sciences, like six sigma, or LEAN methodology

  • Clinical experience, preferred

_License/Certifications_

  • Certification in advanced healthcare quality programs such as CPHA, Six Sigma, or LEAN preferred.

_Position Duties (essential functions denoted with an *)_

  • Supports development, implementation and ongoing improvement of clinical quality initiatives.*

  • Provides one on one direct provider performance feedback.*

  • Provides reports on major variances and quality indicators.*

  • Facilitates teams for planning and development of clinical quality initiatives, patient satisfaction and

  • efficiencies that will lead to cost reduction and waste elimination.*

  • Provides ongoing monitoring information as identified & approved by the CHN Value Performance

  • Committee.*

  • Collects and compiles accurate and timely quality data. Supports CHN quality program and the Value

  • Performance Committee goals in strategic planning and decision-making through assistance in

  • developing, measuring and improving quality outcomes.*

  • Collaborates with peers and practices within assigned region to determine data and system needs,

  • identifying opportunities for synergy and efficiency.*

  • Coordinates data submission to registry and regulatory bodies.*

  • Ensures data collection accuracy via periodic audits.*

  • Ensures regulatory compliance with data submissions.*

  • Disseminates data and feedback to appropriate stakeholders.*

  • Partners with constituencies to evaluate performance of clinical processes and outcomes, customer

  • satisfaction and financial impact of clinical quality decisions.*

  • Works with CHN parties to optimize reimbursement for quality improvement initiative compliance.*
