Under the direction of the Clinical Manager, the Oncology Nurse Navigator assists cancer patients/care givers on their journey through cancer care as an advocate, interpreter, educator, and counselor for oncology patients and their care givers. The incumbent serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations for an extended period of time across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.
_Minimum Education Requirements_
Bachelors of Science in nursing, preferred.
Graduate from a NLN or AACN Accredited nursing program.
High School diploma or GED.
_Minimum Experience Requirements_
5 years experience in oncology nursing in the inpatient and/or outpatient setting, preferred.
Experience/knowledge in community health setting/services, preferred.
Demonstrated proficiency in Microsoft Office Suit, specifically, MS Word.
Current RN license to practice in the state of Colorado
Certification in oncology nursing at time of hire or within 18 months.
_Position Duties (essential functions denoted with an * )_
Demonstrates the knowledge, skill and coordination to provide nursing care and guidance to the cancer patient from first contact and throughout follow-up, serving as an essential link between patients and all other care providers.*
Systematically and continually performs the functions of assessing, planning, implementing and evaluating the care according to the nursing process, Penrose Cancer Standards and ONS Standards of Practice.*
Facilitates the coordination of patient care services to assure excellence in patient care and patient flow by: a) following the patient through the care continuum/experience, eliminating operational (i.e. scheduling, test results, etc) barriers to service; b) working closely with other health care disciplines to ensure timely appointments, results reporting, financial need referrals, communication, patient care and follow-up.*
Interfaces with other healthcare teams for appropriate referrals/services such as the social worker, pastoral care, dietician, dental hygienist, community services, financial counselor, cancer rehabilitation, clinical research nurses, survivorship nurse navigator, palliative care, hospice, smoking cessation, etc.*
Initiates and documents patient teaching including family and significant others based on assessment of needs, including responding to patient request for information regarding the disease process, expected side effects of treatment and community resources. Documents the above appropriately as well as communicating teaching completed to the patients other health care providers as appropriate.*
Continually assesses the patients treatment progress and educational needs, making adjustments to care based on the assessment; consults with primary physician or designee and other members of the care delivery team as needed; refer to appropriate physicians, NPs or programs when needed.*
Collaboratively works with clinical research nurses, facilitating patient referrals for potential clinical trial candidate, communicates patient care needs and schedule with the clinical trial nurse if patient accepted into a trial, negotiates patient care and teaching responsibilities with the clinical trial nurse when the patient is on a clinical trial.*
Active participant on the multidisciplinary care team and conferences, providing specific patient care information pertinent to the patient, follows up on patient care recommendations following the multidisciplinary conferences, and when necessary, facilitates the multidisciplinary conference.*
Facilitate/organize patient support groups as appropriate. *
Serves as a resource for community educational events such as health fairs, screenings. *

Thoracic Nurse Navigator Additional Responsibilities
Coordinates the Pulmonary Nodule Clinic in collaboration with the medical staff.
Coordinates the multidisciplinary thoracic conference, organizing all pertinent patient information, notifying appropriate MDs from pathology, radiology, medical oncology, radiation therapy, pulmonology, cardio-thoracic surgeons and research staff, and follows up on all patient care recommendations made during the conference.
Collaborates with treating physicians, diagnostic testing areas, research staff and other member of the care team to facilitate timely and seamless care
Facilitates patient referrals to smoking cessation programs and resources.
Provides on going communication and coordination of lung nodule patients during the diagnostic phase through follow-up with serial diagnostic studies.
Provides teaching, symptom management and follow up care to patients on Tarceva.
Collaborates with marketing team to promote the Nodule Clinic at PCP offices and community organizations.
Breast Cancer Nurse Navigator Additional Responsibilities
Coordinates the breast cancer multidisciplinary care conference, organizing all pertinent patient information, notifying appropriate MDs from pathology, radiology, medical oncology, radiation therapy, surgeons and research staff and follow up on all patient care recommendation made during the conference.
Coordinate and facilitate the complementary breast group.
Survivorship Nurse Navigator Additional Responsibilities
Develops a patient treatment summary and survivorship care plan for patients that are completing treatment for their cancer.
Meets with patients that have completed or will be completing cancer treatments and/or their caregivers for the purpose of reviewing the treatment summaries, survivorship care plans and make appropriate community referrals as necessary.
Provides patients’ primary physicians with copies of the treatment summary and survivorship care plans.
Coordinate and facilitate the survivorship “cancer transitions” patient group.
