_Position Summary_

To provide for the personal care needs of our clients. To promote and maintain a safe and clear environment for the clients and to serve all clients within the departmental, legal and regulatory guidelines. Requires the employee to demonstrate competency in the direct or indirect provision of individualized care of patients based on the patients assessed age and developmental needs.

_Minimum Education Requirements_

  • High School diploma/GED or equivalent work experience (3 yrs +)

_Minimum Experience Requirements_

  • Preferred: 6 months of CNA experience.


  • Current Colorado Certified Nurse Aide (C.N.A.)

  • CPR = Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED.

_Position Duties (Essential Functions)_

Clinical Practice – 30%

  • Provides and/or assists with personal care needs of patients/residents as directed on Care Plan.

  • Demonstrates clinical competence in the following areas: bathing, vital signs, hair, mouth, and nail care, caring for patients with tubes, foleys, I.V.’s, feeding tubes, assisting with ambulation, exercises, and light housekeeping/meal preparation as assigned.

  • Demonstrates competence in safe equipment use and maintenance.

  • Demonstrates knowledge and compliance with policies.

  • Demonstrates ability to perform duties in variety of environments.

Collaborative Practice – 25%

  • Communicates with nurse regarding patient’s condition.

  • Participates in patient care conferences.

  • Communicates need for Supervisory Visit to primary nurse and assists with coordinating visit.

  • Participates in quality processes through reporting of Customer Concerns and Occurrences and participation on work teams.

  • Notifies clinical leadership of any environmental concerns.

Documentation – 25%

  • Submits DAR’s (complete) along with paperwork per agency timelines.

  • Documents care provided (or reason why visit not made) in correct format.

  • Visit frequency and duration is in compliance with physician orders. Supports operational needs related to documentation requirements both internally and externally.

Professional Accountabilities – 20%

  • Attends all mandatory/scheduled meetings.

  • Maintains productivity per FTE status.

  • Adherence to Centura Health Associate Principles/Human Resource policies regarding dress code, attendance, punctuality etc.

_Physical Requirements_

  • Medium Work - exert/lift up to 50 lbs force occasionally, and/or up to 20 lbs frequently, and/or up to 10 lbs constantly.
