Under the direction of the Director of Oncology and the Medical Director of the Cancer Program, the RN Navigator will assist cancer patients on their journey through cancer care as an advocate, educator, coordinator and clinical liaison. In conjunction with the Medical Director of the Oncology Program, the Nurse Navigator will: assist in developing protocols, follow the care of each patient, coordinate staff education, collect data and assure that the breast center accreditation designation is maintained. Will assist in leading the multidisciplinary conference and clinic as well as develop relationships with other service providers.
Minimum Education Requirements
Graduate from an accredited nursing program.
Bachelors of Science in nursing preferred
Minimum Experience Requirements
2 years of experience in oncology nursing in the inpatient and/or outpatient setting preferred
Experience/knowledge in community health setting/services desirable
Current state RN license.
Certification required within specialty, preferred.
Current Basic Life Support certification from American Heart Association.
ACLS and PALS required (Perioperative only).
Position Duties (essential functions denoted with an * )
Serve as an essential link between patients and all other care providers.*
Facilitate appointments for consults and support services within established service standards, including social
workers, dietitians and physicians.*
Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care
Provides psychosocial support including facilitation of support groups*
Coordinates the diagnostic work-up and treatment plan with the multidisciplinary team.*
Maintains consistent and regular communication with the cancer patient pre and post operatively*
Confirm positive biopsy results with radiologist/ pathologist, and transition care for patients with positive biopsies
from radiology or positive pathology report to the cancer program*
Make phone calls to patient and referring physician for all positive biopsies to ensure patient has follow-up care.*
Be available to triage/manage active patient issues for practices*
Uses designated software application for patient documentation, tracking, and survivorship as designated by
Develop patient education programs and tools; and keep current*
Identifies and provides education to patient/families about current and potential health problems*
Provides formal and informal education to other members of the health care team to achieve desired outcomes*
Coordinates and participates in community education programs, health fairs and local events.*
Demonstrates the knowledge, skill and coordination to provide nursing care and guidance to the cancer patient
from first contact and throughout follow-up.*
Demonstrates advanced clinical and managerial skills contributing to the development, maintenance and change
in the delivery system for the care of the cancer patient.*
Coordinates with marketing*
Share with COC and BPL; develop community outreach standards.*
Works in conjunction with the Director of Oncology Services and Medical Director, surgeons, med oncologists,
radiation oncologists, pathologists, radiologists, nurse managers, and clinicians to assure excellence in clinical
Collaborates with physicians, oncology support staff, business development staff, and other clinical members to
assist with the implementation and marketing of the comprehensive cancer programs/services.*
Supports growth initiatives of the cancer program and actively participates in such tactics*
Assists physicians in developing and revising clinical practice guidelines for breast care*
Incorporates appropriate research finding into clinical practice*
Maintains ongoing analysis of the program in conjunction with the Medical Director and Director of Oncology,
providing outcome measures to assess efficacy and performance improvement of the program.*
Maintains up to date database for providing program reports.*
Organizes implements and attends the multidisciplinary Breast conference and documents the treatment plans
developed for patients and tumor boards with the Cancer Registrar.*
Participates in determining goals/objectives/strategic plan for the Breast Program and Cancer program.*
Participates in professional organizations, attends professional workshops/conferences*
Is actively involved in maintaining Breast Center accreditation and network cancer accreditation.*
Participates in cancer committee and BPL in member and patient navigator and quality role.*
Is actively involved in maintaining Commission on Cancer accreditation.*
Works collaboratively with cancer registry on patient cases, numbers, quality, pathology, etc. *
